A dementia patient may not be able to answer basic medical questions under the best of circumstances, when they are sick or injured it will probably be impossible.  

Emergencies have a way of making us nervous, possibly even scared.  We are likely to forget things in these circumstances.  

Don&#39;t take the chance of forgetting to tell your loved one&#39;s doctor about all the medication she takes or the hearing aids he wears.

Mistakes or omissions in reporting medical history can lead to unnecessary tests or even harmful treatments.  Record your loved one&#39;s medical history BEFORE you need it.</description>
